Position Description

Summary: Coordinates the services between DSIG/ThriveRx, and referral sources and patients for intravenous nutrition and other infusion medications as needed. Includes initial referral coordination and patient training, hospital-based rounding to coordinate discharge and education of hospital referral sources regarding services offered. Assists with educational and supporting material development.

Education/Experience:  Experience with intravenous therapy administration and patient education. Ability to self-organize and prioritize field customer service activities. Prior experience with customer service required.  Flexible communication style, highly motivated and excellent listening skills required. Registered Nurse with home infusion experience preferred.

Certificates and Licenses:

  • Current state licensure as a registered nurse.
  • CRNI and/or PICC Certification preferred.
  • CPR certification required.

Duties and Responsibilities: Other duties may be assigned.

Technical/Clinical

  • Coordinates hospital discharge with case manager, hospital team and patient/family.
  • Obtains clinical documentation and orders required for reimbursement purposes from case managers and/or physician office.
  • Works with DSIG intake team, pharmacy and nursing to onboard new patient referrals.
  • Assess and teach patients on home nutrition support and other medication administration, as needed, during initial visit and ongoing, as needed.
  • Communicates care plan and patient visits with home health agencies, as needed.
  • Participates as necessary in internal and external patient care conferences.
  • Completes clinical rounds at designated hospitals on an ongoing basis to follow-up on hospitalized patients and to solicit new referrals.
  • Timely and appropriate documentation in the patient medical record, as indicated.
  • Keeps current with infusion nursing research and regulations.

Program Development:

  • Assist with development of clinical standards, policies and procedures and clinical forms.
  • Develops and ensures appropriate distribution of consumer/patient educational materials.
  • Works with Account Manager to create and implement a territory marketing plan to include;
  • in-service education to hospital and physician office staff to promote clinical education and DSIG marketing.

Communication and Reporting:

  • Able to effectively communicate with DSIG team, medical team members and patients, following acceptable communication methods and consent documentation.
  • Maintains confidentiality of patient and competitive information.
  • Completes required reporting and accounting of time, expenses and other reports as required, within the stated timeframe.
  • Demonstrates a high degree of personal responsibility regarding accuracy and quality of work.

Language Ability:

Ability to comprehend and apply principles of basic math while analyzing data and generating reports.   

Reasoning Ability:

Ability to apply principles of logical thinking to a wide range of intellectual and practical problems. 

Computer Skills:

To perform this job successfully, an individual should have knowledge of Microsoft products.
